We are looking for a positive, enthusiastic and dynamic social work practitioner to work as part of the care coordination team in all Claire House settings; Wirral Hospice, Liverpool Hospice and the community setting.

This role is full-time and can be worked flexibly across 4 days, a minimum of 30 hours is required, predominantly office hours, with an expectation to work flexibility over a 7-day period if needed, with an option to work at home 1 day a week; dependent on need.

This role involves supporting families holistically within their journey at Claire House children’s hospice. This role will be a main point of contact for families and professionals to build positive relationships, with a specific lead for social aspects of care and support.

Key skills for this job include: Experience working with children/young people and families, excellent communication skills, organisational skills, ability to work in a small team, ability to work autonomously, able to prioritise and manage time and workload, ability to problem solve, adaptable, car driver.

This role will provide specialist resource in social care knowledge with a strong understanding of child and family social work principles, theories and practices.
