Care Coordination Team - Social Worker (Caseload Holder)
Salary £37,200 – £44,700 (Band 6)
We are looking for a positive, enthusiastic and dynamic social worker role to work as part of the care coordination team in all Claire House settings; Wirral Hospice, Liverpool Hospice and the community setting.
This role is full-time and can be worked flexibly across 4 days, a minimum of 30 hours is required, predominantly office hours, with an expectation to work flexibility over a 7-day period if needed, with an option to work at home 1 day a week; dependent on need.
This role involves supporting families holistically within their journey at Claire House children’s hospice. This role will be a main point of contact for families and professionals to build positive relationships, with a specific lead for social aspects of care and support.

This role will provide specialist resource in social care knowledge with a strong understanding of child and family social work principles, theories and practices.
